In terms of generic, but specific advice applicable across multiple projects, here are some foundations on which a process of evolutionary innovation can be built:

--professional development based on face-to-face and virtual communities of practice (see Wenger's work in this area at http://www.ewenger.com/)

--planning based on Peter Senge's model of learning organizations (see his recent book on Schools that Learn)

--linking research and practice in ways that empower both (see my recent Congressional testimony at http://www.house.gov/science/research/reshearings.htm)

This is a good overall strategy for moving beyond project sustainability to inculcating a culture of evolutionary innovation throughout an organization
